Form query
RPSL Form III, Form V, Form VI and Form VII guide
The forms should match the company, signatories, manning agreement and vessel evidence.
Why forms matter
RPSL forms are often treated as clerical attachments. In practice, they become the spine of the application. If the form details do not match the company records, signatory list, shipowner documents or proposed vessels, the file can receive queries.
Common form mismatch areas
Before upload, every form should be checked against the underlying document set.
- Authorized signatory count and names
- Company address and office evidence
- Shipowner, employer and vessel details
- Number of seafarers and bank guarantee slab
- Technical personnel or responsible person information
How we review forms
We review the forms as a connected set, not standalone PDFs. That means checking consistency against company records, agreements, CBA/SEA wording, vessel documents and supporting declarations.
Frequently asked questions
Can RPSL forms be revised after query?
Yes, authorities may ask for revised forms or corrected uploads. However, repeated corrections can slow the file and should be avoided through pre-upload review.
Do form details need to match Form III exactly?
Where signatories, company details or declared responsibilities overlap, the details should be consistent across the forms and supporting records.
